ABSTRACT

【Objective】 To investigate the association of depressive symptoms with the predicted risk of coronary heart disease in middle-aged and elderly Chinese based on a large community study. 【Methods】 A total of 2532 cases in the group without depression and 2758 cases in the group with depression were included. We compared the two groups in general demographics, information related to coronary heart disease risk, and physical function and ability to perform daily living. We also analyzed the factors associated with coronary heart disease risk by linear regression. 【Results】 ① Demographic information: The group with depression had a higher mean age, a higher proportion of women, more people with poor marital status, and a higher number of comorbid chronic diseases compared with the group without depression (all P<0.05). ② Risk indicators related to coronary heart disease: The group with depression had more people with diabetes and a significantly higher systolic blood pressure compared with the group without depression (P<0.05). The two groups did not significantly differ in the proportion of smokers, diastolic blood pressure, LDL-C, or HDL-C (all P>0.05). The risk of coronary heart disease was significantly higher in the group with depression than in the group without depression (P<0.05). ③ Physical function and ability of daily living: The physical function score, physical self-care score, and instrumental daily living ability were significantly higher in the group with depression than in the group without depression (all P<0.001). ④ Linear regression showed that except for gender, age, marital status, comorbid diabetes, smoking, systolic and diastolic blood pressure, HDL-C and LDL-C were associated with risk of coronary heart disease (P<0.05); CESD was the only factor associated with the risk of coronary heart disease [B=0.019, 95% CI: (0.015, 0.032), P=0.032]. 【Conclusion】 The risk of coronary heart disease is higher in middle-aged and elderly people with depressive symptoms than in those without depressive symptoms. Having depressive symptoms is one of the risk factors for coronary heart disease in middle-aged and elderly people.

ABSTRACT

Objective:To analysis the prevalence and characteristics of comorbid insomnia and sleep disordered breathing (SDB) in community population. Methods:This is a cross-sectional study. Cluster sampling was applied. Community residents in Shantou and Meizhou were investigated during April to May, 2021. Essential information collection, sleep related health investigation, and sleep study were conducted. Insomnia was defined as the insomnia severity index (ISI)≥8. A type Ⅳ wearable intelligent sleep monitor was applied for sleep study. Comorbid insomnia and SDB was defined as both diagnosis of insomnia and SDB. Statistical analysis was conducted by SPSS 25. Prevalence and characteristics of insomnia only, SDB only and comorbid insomnia and SDB were analyzed. Logistic regression was conducted to analyze the relationship between sleep disorders and unrestored sleep, abnormal glucose metabolism, hyperlipidemia, and cardiovascular disease. Results:There were 3 730 residents completed the investigation. The median age was 55.0 (46.0, 63.0) years. The prevalence of insomnia only, SDB only, and comorbid insomnia and SDB were 26.0%, 20.2%, and 10.4% respectively. The incidence of unrestored sleep in insomnia only and comorbid insomnia and SDB were 2.900 times and 3.777 times of that in no insomnia or SDB (both P<0.001); the risk of hyperlipidemia was elevated in insomnia only, SDB only and comorbid insomnia and SDB ( OR=1.553, 1.415, and 1.868; all P<0.05); the risk of cardiovascular disease increased 40.8% in SDB only ( P=0.001), and 42.1% in comorbid insomnia and SDB ( P=0.007), after adjusted by age, sex, body mass index (BMI), smoking, drinking, abnormal glucose metabolism, and hyperlipidemia. Stratified analysis revealed that young female (age<60 years) with normal BMI (<25 kg/m 2) and comorbid insomnia and SDB were associated with higher risk of cardiovascular disease. Female with normal body weight and insomnia only or comorbid insomnia and SDB were associated with higher risk of hyperlipidemia. Conclusions:The prevalence of comorbid insomnia and SDB is high in community population. Patients with comorbid insomnia and SDB present with more significant unrestored sleep, and are correlated with higher risk of hyperlipidemia and cardiovascular diseases.

ABSTRACT

Objective:Explore the relationship between sleep duration, sleep time and brachial-ankle pulse wave velocity(baPWV) in community population. Methods:Questionnaire, physical examination, blood tests, and baPWV detection were applied to a community based population. Finally, 3 912 subjects with complete data were included in the study. The relationship between sleep duration, time to fall asleep and PWV was evaluated with binary logistic regression analysis. Results:Being adjusted for age, sex, prevalence of diabetes, sleep condition, body mass index, blood glucose, blood pressure, dyslipidemia, ankle-brachial index, sleep duration and time to fall asleep were correlated with PWV. The risk of PWV abnormalities was increased in the≥8 h group compared to the 6-8 h group( OR=1.155, 95% CI 0.995-1.367, P=0.037). The risk of abnormalities PWV was higher in the group with sleep time after 00: 00 than in the group -23: 00( OR=1.482, 95% CI 1.008-2.179, P=0.045). Conclusion:Long sleep duration(≥8 h) and late sleep time(after 00: 00) may be associated with higher risk of atherosclerosis.

ABSTRACT

Objective:To analyze the influenza vaccination status of chronic disease management patients in Qingpu district of Shanghai and the vaccination characteristics of different characteristic populations, so as to provide scientific basis for improving the influenza vaccination rate of chronic disease patients in the community. Methods:By comparing the data of Shanghai chronic disease management information system, immunization planning information system and medical association platform, 89 453 subjects who met the inclusion and exclusion criteria in Qingpu district were selected as the research objects. The vaccination coverage rate of the study subjects was calculated according to gender, age group, urban and rural distribution, occupation, chronic disease type and quantity, and the vaccination coverage rate of different subgroups was compared to analyze the influencing factors of vaccination coverage rate. Results:Most of the 89 453 patients with chronic diseases were 60 years old and above (71.93%). Patients with hypertension, diabetes, chronic obstructive pulmoriary disease (COPD) and three chronic diseases accounted for 87.12%, 28.67%, 8.71% and 1.83%, respectively. Influenza vaccination coverage in the 2016/2017 flu season was low, at 0.32%. Influenza vaccination coverage rate of women (0.37%) was higher than that of men (0.27%), which was 1.41 times respectively(95% CI: 1.16, 1.72) that of men patients. The coverage rate of influenza vaccination for the 70-79 year-old group was the highest (0.74%), which was 1.74 times respectively(95% CI: 1.39, 2.19) that of 60-69 year-old patients. The vaccination coverage rate of government departments and institutions was the highest (1.14%), which was 12.58 times respectively(95% CI: 4.52, 34.99) that of retirees. The vaccination rate of COPD patients (3.68%) was 2.50 times (95% CI: 1.66, 3.77) higher than that of patients without COPD.Conclusions:Influenza vaccination rate for chronic disease management patients in Qingpu district of Shanghai is low. Gender, occupation, age and types of chronic diseases are the influencing factors. Patients with chronic disease management should be included in the priority vaccination targets for influenza vaccines, and vaccination intervention for occupational chronic diseases such as non-retired agriculture and forestry patients, especially male patients, should be strengthened to improve influenza vaccination coverage rate.

ABSTRACT

Objective@#This study aimed to analyze and summarize the implementation of the primary prevention program for stroke in Shanghai rural community and evaluate its preventive effect.@*Methods@#A cluster sampling method was used to select approximately 50 000 community natural population from a town in Fengxian District of Shanghai. A primary stroke prevention program was established and implemented in the community from 2008 to 2010 and from 2012 to 2015. The prevention program inclueded stroke health education for the whole population, screening of high risk population of stroke, and therapeutic intervention and management plan for individuals aged≥50 years. The intervention measures were followed up every 2 months. From 2008 to 2015, new stroke cases were monitored dynamically and reviewed annually with classification of stroke diagnosis. The effect of community primary prevention of stroke program was evaluated by the dynamic changes in stroke incidence.@*Results@#From 2008 to 2015, a total of 422 600 individuals were monitored, including 20 500 men and 217 600 women. In 2008, 7 828 individuals were screened and 2 129 individuals were at high risk of stroke were screened, with a high risk rate of 27.2%. In 2012, 2 877 individuals were screened, and 725 individuals were at high risk of stroke were screened, with a high risk rate of 25.2%. The standardized incidence of stroke in this community was 158.1, 105.4, 86.6, 77.6, 89.3, 77.4, 50.7, and 42.3 per 100 000 respectively. The overall incidence image roughly showed a downward trend except 2012(trend test P<0.01). There was no significant difference in the incidence of stroke between men and women in each year. The incidence of stroke increased significantly with age (trend test P<0.01). The age composition of stroke patients is slightly different between different sexes. The proportion of male patients with stroke aged <70 years was significantly higher than that of female patients with stroke, while the proportion of female patients with stroke age>70 years was significantly higher than that of male patients with stroke (P<0.01). Cerebral hemorrhage, cerebral infarction, and subarachnoid hemorrhage accounted for 20.76%, 75.14%, and 4.10%, respectively.@*Conclusion@#Based on lifestyle intervention and treatment and control of risk factors, screening high-risk individuals for therapeutic intervention in community population can significantly reduce the incidence of stroke.

ABSTRACT

Objective To investigate the relationship between sleep duration (SD) collected by polysomnography and ischemic stroke in community populations. Methods We recruited 2062 participants, including 724 males and 1338 females. The mean age of the all participants was 61.44 years. In this study, the subjects were divided into three groups according to SD: short SD (8 h), and normal SD (6-8 h). Logistic regression was used to analyze the independent association between the three different sleep durations and the occurrence of ischemic stroke. Results ① The three groups differed significantly in gender, age, BMI, prevalence of diabetes and serum triglyceride levels (P<0.05). ② The incidence of ischemic stroke was significantly higher in short SD group than in long SD group and normal SD group (the incidence rates were 5.9%, 2.7% and 1.6%, respectively; P=0.044, P<0.001). ③ Univariate logistic regression analysis suggested that short SD was associated with the occurrence of ischemic stroke (OR=3.845, 95% CI: 1.789-8.266, P=0.001). After adjusting potential factors such as gender, age, marital status, alcohol consumption, hypertension, diabetes and triglycerides, multivariate logistic regression analysis showed that short SD was still an independent risk factor for ischemic stroke (OR=2.629, 95% CI: 1.033-6.693, P=0.043). Conclusion The incidence of ischemic stroke differs in people with different SD. Among them, the incidence of ischemic stroke is higher in people with short SD.

ABSTRACT

Objective To investigate the prevalence of traumatic event exposure and its relationship with major depressive disorder (MDD) in community population in Tianjin. Methods A multi-stage cluster random sampling method was used for the general health questionnaire (GHQ-12) in the population aged above 18 in 15538 families of 18 counties in Tianjin city. Among the 11748 subjects who completed the survey, they were divided into the high-risk group (100%), medium risk group (40%) and low risk group (10%) according to the risk level of mental disorders. Finally, 4438 residents were interviewed with the diagnostic and statistical manual of mental disorders Fourth Edition (DSM-Ⅳ) axisⅠdiagnostic clinical interview version (SCID-Ⅰ/P). The prevalence of respondents exposed to traumatic events and MDD was calculated. Logistic regression analysis was applied to obtain the risk factors of MDD. Results A total of 469 people experienced traumatic events in 4438 subjects, and the adjusted total exposure rate was 10.3%. Seventy-six people suffered from MDD in 469 cases with traumatic events, and the adjusted total exposure rate was 6.7%. The prevalence of MDD was 3.3% in people with no traumatic events. Logistic regression analysis showed that female (OR=1.77,95%CI:1.27-2.46), widowed/divorced and experienced a traumatic event (OR=2.21,95%CI:1.46-3.34) were the risk factors of MDD. Conclusion The total exposure rate of traumatic events is 10.3% in community residents in Tianjin, and the prevalence of MDD is related with the traumatic event exposure. It is suggested that the whole society should mobilize and participate extensively, and pay more attention to the psychological health problems of high risk population.

ABSTRACT

The residents who had lived for at least 5 years and aged over 20 years old were sampled from urban to rural districts of Jiangsu Province with a stratified cluster sampling technique. B mode ultrasonography and thyroid function determination were carried out in 6 128 persons. The location, diameter, number, boundary, and calcification in thyroid nodules were described by using 7.5 MHz/50 mm transducer of thyroid ultrasonography. TSH was measured by chemiluminescence immunoassay. Free triiodothyronine(FT3)and free thyroxin(FT4)were measured when TSH was abnormal. The crude prevalence of thyroid nodules was 21.12% in total population, 14.55% in male, and 25.24% in female. The standardized prevalence was 15.69%, 11.20%, and 20.40%, respectively. The prevalence was lower in male than in female, and increased with age(P<0.05). Thyroid nodules in Jiangsu Province were highly prevalent and more attention should be paid to the follow-up, early diagnosis, and treatment.

ABSTRACT

Objective To investigate the effect of commercial low-sodium and high-potassium salt substitutes on blood pressure in the rural community-based population in China. Methods We conducted a quasi-experiment on 411 adults, who were 30 to 60 years of age, in 2 rural communities from Laiwu city in Shandong province of China on data from blood pressure screening. The subjects were divided into 2 groups: high blood pressure (HBP) and non-HBP (NHBP). Both groups and their family members took a low-sodium and high-potassium salt substitute for 3 months to replace the normal salt in their bodies. Blood pressure (BP) and 24-hour urinary sodium and potassium were measured regularly in the 2 groups. Results There was a continuously decreasing trend for BP at the end of the first month. Three months later, the mean BP decreased by 7.4 mm Hg (1 mm Hg=0.133 kPa, t=10.096, P=0.000) for SBP and 3.8 mm Hg (t=8.017, P=0.000) for DBP in the HBP group, when compared to a 1.2 mm Hg(t=2.507,P=0.007) decrease on SBP and 1.0 nun Hg(t=2.987, P=0.002) on DBP in the NHBP group. The mean urinary sodium had a decrease of 15.5 mmol/24 h (t=1.803,P=0.037) ,but the urinary potassium increased by 4.2 mmol/24 h (t' =2.132, P=0.018). The result of urinary sodium appepared to be as follows:potassium ratio (Na+/K+ ) decreased by 1.2 (t=2.786,P=0.003) in the HBP group. However,in NHBP group, the mean urinary sodium decreased by 1.7 mmol/24 h (t=0.211, P=0.417) and urinary potassium increased by 3.7 mmol/24 h (t' =2.207,P=0.015) ,together with the decrease ofNa+/K+ by 0.7 (t=1.818, P=0.036). Conclusion Results from our study clearly demonstrated that the intake of low-sodium and high-potassium salt substitute could effectively reduce the BP with good compliance among adults in the rural community-based population in China. This was an effective but non-medical method to prevent and control the high blood pressure.

ABSTRACT

Objective The aim of this study was to investigate the relationship between obesity [measured by body mass index(BMI), waist circumference(WC), waist-to-hip ratio(WHR)]and arterial stiffness on community populations in Beijing area. Methods In a cross-sectional study of 2664 subjects(1379 men and 1285 women)aged(53.19 ± 15.73 years, mean±standard deviation), BMI, WC, WHR and other cardiovascular risk factors, were measured and carotidfemoral pulse wave velocity(cfPWV)was measured to assess the arterial stiffness. All the data were analyzed with linear correlation analysis, univariate analysis and stepwise regression method to explore the relationship between obese indexes and arterial stiffness. Results We noticed that a showed the existence of positive correlations between BMI(r=0.0829,P<0.01)and cfPWV, WC(r=0.2659, P<0.01)and cfPWV, WHR(r=0.2749, P<0.01)and cfPWV, in univariate analysis,cfPWV was associated with WC≥85 cm(male)or ≥80 cm(famale)(P<0.01), WHR≥0.90(male)or≥ 0.85(famale)(P< 0.01). Stepwise regression analysis revealed that WC was an independent risk factor of cfPWV, other than age, gender, systolic blood pressure, HDL-C, OGTT2h etc. Conclusion Our findings indicated that obesity was an independent risk factor of cfPWV, which was an early marker of cardiovascular and renal diseases, among community population in Beijing area. There were different relationships between obesity measurement parameters(BMI, WC, WHR)and cfPWV.

ABSTRACT

Objective To explore the association between metabolic syndromes (MS) and carotid atherosclerosis and to estimate the predictive effects of MS under 3 different definitions. Methods A cross-sectional study was conducted in 2 community-based populations in Beijing,in 2008.1266 subjects (598 men,668 women),aged 45-69,were included in the analyses.MS was defined by the criteria of International Diabetes Federation (IDF),the revised NCEPATPm (ATP Ⅲ-R) and "The Guidelines of Dyslipidemia Control for Chinese Adult" ( "Guidelines" ) in 2007.Results The prevalence rates of MS by the 3 criteria were 39.0%,43.3% and 30.9% respectively. The Kappa value for the measure of the agreement between each pair of the 3 definitions were 0.911,0.719 and 0.730 respectively. The intima-media thickness in common carotid artery (CCA-IMT) was significantly higher (P<0.001) in all MS groups than in non-MS groups, diagnosed with the 3 criteria independent of age,gender,LDL-C,and current smoking status. After adjustment of age,gender,LDL-C,and current smoking status, the classification of MS significantly increased the risk of prevalence of carotid atberosclerotic plaques, compared to the non-MS group.OR value were 1.499 (95% CI:1.157-1.942) for IDF,1.696 (95% CI:1.314-2.189) for NCEP-R,1.763 (95% CI:1.344-2.312) for "Guideline" respectively. Conclusion Our research findings indicated that, when MS were defined with the 3 definitions, prediction on the risk of sub-clinical atberosclerosis would work beyond some of the conventional cardiovascular risk factors such as smoking,LDL-C.There might exist some differences in gender issue on the strength of association between MS when diagnosed by different criteria and carotid plaque.

ABSTRACT

Six thousand and forty-four subjects in Jiangsu community were enrolled to investigate the relationship between subclinical thyroid dysfunction and blood pressure. It was shown that subclinical thyroid dysfunction, including both the subclinical hyperthyroidism and subclinical hypothyroidism, had no relationship with increased blood pressure.
